LENSAR Laser System

The LENSAR Laser System combines the specially developed automatic biometrics, expanded ocular imaging and measuring technology 3D-CSI (confocal structured illumination) with an Advanced Ultra Short Pulse Laser.

By means of a maximum of automated steps, the system was designed to provide manual incisions and reduced ultrasound need in all degrees of a cataract, to offer a new, unprecedented level of precision for the refractive cataract procedure. The system performs all CCIs, LRIs, capsolutomies and lens fragmentations fully automatically after a 3D image has been created and a measurement of all ocular surfaces and structures, starting from the cornea’s surface to the back of the crystalline lens, has been carried out.

The system is very variable and adapts well to different OR environments and conditions. It can be used directly in the operating theatre to perform complete treatments in a room or in an anteroom of the operating theatre.

Thanks to the system’s unique design and dimensions with its retractable laser console and motorized substructure, the doctor can freely choose his own position in relation to the patient's head. The system requires only minimal movement of the patient, because the laser moves to the patient and not the patient to the laser.

Easy patient docking

The "no corneal touch" patient interface, specially developed for this application and the servo-controlled docking system lead to a very low increase in intraocular pressure and thus to an increased well-being of the patient.

The doctor's excellent viewing angle of the patient's eye enables perfect observation of the treatment for the surgeon and the surgical staff. The two-part system is easy for the doctor to operate and dock. The suction ring is positioned on the sclera and leaves the cornea untouched. Subsequently, the laser console and servo-controlled docking system is moved by the doctor using the joystick. With the help of an aiming system, the eye is safely docked, imaging is started and treatment can begin.

Highest precisions thanks to the 3D-CSI measurement and image processing

3D-CSI is a unique, automated measurement and image processing technology that enables the visualization of all relevant surfaces of the frontal eye segment from the frontal corneal surface to the posterior capsule surface of the lens.

From this image information, all surfaces are automatically localized, measured and biometrics are generated. Image processing uses the Ray Tracing principle to automatically create a true 3D reconstruction of the frontal eye section and then automatically generates the best treatment pattern for each individual eye.

CSI is short for “Confocal Structured Illumination”. The beam paths for the laser and scan optics are identical to ensure that the system uses the same optical path. This makes it possible to work with the highest precision and also to align the laser beam exactly at the spot that was previously scanned by the scanner.

This self-developed system varies the scanning rates, creating four different lighting zones to achieve high quality images of surfaces with different reflective properties. These zones are: the central area of the cornea, the outer corneal margin with limbus (taking into account the strong reflection of the iris), the frontal lens surface and the posterior lens surface while considering the different degrees of lens opacity.

That way, the system is able to produce the highest quality images with the greatest possible depth of field and allows the image processing software to automatically generate the 3D reconstruction and the necessary treatment patterns for the entire treatment.

Software-supported capsulotomy and lens fragmentation

As the 3D-CSI diagnostic system has generated a biometric reconstruction of the frontal eye section, the software can now offer automatic positioning options for the size, shape and position of the capsulotomy according to the eye’s real anatomy.

The treatment algorithms were optimized for each degree of cataract and the system fragments the lens with maximum efficiency. This leads to a significant reduction or even elimination of the required ultrasonic energy to complete the cataract procedure.

The 3D-CSI diagnostic system significantly improves the efficiency and safety of the procedure by automatically adjusting for any inclination and de-centreing of the eye lens that may be anatomical or treatment-related.
